In order to receive Live Scan Fingerprint Service, you need to bring the following to your appointment:
1. A Request for Live Scan Service form (BCII 8016).
It will be either a triplicate form (carbonless copy paper) or you
print 3 copies of the form from the internet. If you were
provided with a one-page form, photocopy it twice so you have a total
of 3 copies. Bring all 3 copies. Fill out the form before
your appointment. Your Employer or Requesting Agency should
provide this form to you. Other names for this form are RE 237, DMV 8016, SOS/BCII 8016.
2. One form of valid Photo ID, such as a Driver's License, Passport, or Military ID. Make sure your Photo ID has not expired!
3. Payment -- We accept Visa, MasterCard, Debit, Check, Money Order, and Cashiers Check. Sorry, No Cash.
If your Employer or Requesting Agency provides a Billing Number on
your form, then you will only be responsible for paying the Rolling
Fee. Otherwise, you are also responsible for the Government Fees indicated on your form.
Filling out the Live Scan Service Request form (BCII 8016)
Your Employer / Requesting Agency should provide the following information to you.
1. ORI (comprised of a letter followed by four numbers, examples: A0075, X0881)
2. Type of Application
3. Job Title
4. Agency Address Set Contributing Agency 5. Mail Code (if applicable, comprised of five numbers)
6. Billing Code (if applicable, comprised of six numbers)
7. OCA (if applicable)
8. Level of Service (DOJ / FBI)
Fill out Applicant Information before your appointment.
1. Name
2. AKA's (Also Known As; aliases; other names you've used)
3. CDL No. (California Driver's License; if presenting an out-of-state
Driver's License, Military ID, or Passport, write these under Misc. No.)
4. DOB (Date of Birth)
5. Sex (Male or Female)
6. Height
7. Weight
8. Eye Color
9. Hair Color
10. POB (Place of Birth)
11. SOC (Social Security Number)
12. Misc. No. BIL- (Billing Code provided by Employer / Requesting Agency, if applicable)

Ink & Paper Fingerprint Service
In order to receive Ink & Paper Fingerprint Service, bring the following to your appointment:
1. One form of valid photo ID such as a State Driver's License, Passport, Military ID, or Greencard.
2. Fingerprint card such as the FBI FD-258 hard card. Fill in your personal information.
3. Payment by Visa, MasterCard, Debit, Money Order or Check. No Cash please.
